Scoliosis
The scoliosis is a sinuous deformation of the Spinal column in the frontal plan with rotation of the Vertèbres in the horizontal plane. There are one or more deviations in the three plans of space.
Differential diagnosis
The scoliosis is known as “true” (or structural) when a rotation is associated with the side deviation; this case relates to only one small proportion of the general population. The great majority of the scolioses of the Enfance are “idiopathic”, which means that their origin is unknown, unlike the secondary scolioses to an anomaly neuromusculaire, a tumor, a traumatism or a vertebral malformation. The idiopathic scoliosis is to be also differentiated from the scoliotic attitude, or postural scoliosis, which does not comprise rotation of the vertebral bodies, and which is corrected in position lying. It can also be hereditary.
Evaluation
There exist four principal types of scoliosis:- thoracic (dorsal)
- thoraco lumbar (dorso-lumbar)
- lumbar
- double
The principal criterion of evaluation of the scoliosis is the angle of Cobb, which is measured on a radiography of the rachis. It is the angle formed starting from the two tangential line intersection one with the higher plate of the vertebra limits higher, the other with the lower plate of the vertebra limits lower.
Natural history
The idiopathic structural scoliosis usually develops after early childhood, before the Puberté and during the puberty growth. It reaches in majority the female population (once and half with twice as much as at the boy).
The long-term consequences of the idiopathic scolioses are:
- the respiratory gene, with reduction in the Vital Capacity (CV), in VEMS. The reduction in the CV is severe when the dorsal scoliosis exceeds 90°.
- the rachidian pains more frequent than compared to a reference group
- the daily life is disturbed, but rather during the periods of rachidian pains aiguës.
Les rachidian pains and the disorders respiratory is all the more important as the angle of Cobb is large.
Not operated, the idiopathic scoliosis has a variable repercussion on quality of life, according to its sévérité.
Lorsque angle of Cobb is lower than 30° at the end of the growth, it is rare that it worsens later. When it is higher than 50°, the respiratory disorders are all the more important as this angle is high.
Treatments
The treatments aim limiting the natural progression of the scolioses, in particular towards the serious forms, and at reducing (evolution towards and) the respiratory and painful complications. The indications are variable. One has of the Kinésithérapie, the port of the Corset and as a last resort the Chirurgie.
