Rosacée (pathology)

The rosacée , also known under the name of blotches is an incurable cutaneous disease at the beginning benign which appears by chronic rednesses on the level of the nose, cheeks, sometimes also on the level of the chin and face. These symptoms are accompanied by a feeling of tingling, in particular on the level of the eyes.
De small blood-vessels are often visible in the zones touchées.
The disease is evolutionary and can cause crises of Acné, in particular in the event of stress or of tiredness, and, at a advanced stage, to make swell the nose of the subject which remains red and embossed (rhinophyma).
Lorsque ocular itchings are very strong, rosacée can also hard affect the sight of the subject which in is reached and an ophthalmologic examination is recommended.

Technical explanation

The medical research is not completely fixed on the precise process and even less on its causes, but one can make a description sommaire.
of it When a subject reached of rosacée is subjected to an environmental factor release (temperature, particular food, emotion…), a vascular phenomenon engages: blood circulation increases clearly in the zones concernées.
Certains irritating factors involves an accumulation of important liquid and causes a puffiness and a chronic local dysfunction of the lymphatic system.
L' exposure to the sun, cold, with the wind, can damage this same lymphatic system by causing a degeneration of the elastin.
La inflammatory reaction causes Papule S and pustules.
Des dilated blood-vessels (Télangiectasie S) can appear on the surface of the skin and are the symptom of a advanced state of the maladie.
Rosacée is often accompanied by skin troubles such as the séborrhéïque parakératose, the Eczéma or the Psoriasis.

Research recently wondered about the supposed role of two parasites, Demodex folliculorum and Demodex brevis, of the Acarien S which seem to be present in very great quantity in the skin of the people reached of rosacée and who are suspected of being about it partially the cause.

Sub-types of rosacée

It is noticed well that the symptoms of the blotches, rosacée, and the seborrheic dermatite are often recut, just as their releases (as long as one is not in the extreme of each one of these diseases). It is completely normal to have a little blotches (some small visible blood-vessels), especially after the 30 years age. That does not want to however say that one is reached of blotches! Those and those which have the rednesses accompanied sometimes by a small eczema behind the ears (or which had eczema elsewhere on the body during their youth) should perhaps see whether it rather do not have small a dermatite seborrheic (problem which can be regulated temporarily into 3 to 7 days!). Perhaps several have the red cheeks but not acne (unless eating a ton of chocolate and 3kg of curry!). Perhaps in this case one should not seek side of rosacée but well of a seborrheic dermatite. One recognizes this problem by pores of the skin more dilated and… the production of oil at certain places of the face or face! With the seborrheic dermatite, a simple application of cream of hydrocortizone 0.5% or 1% available in pharmacy is amply enough to reabsorb rednesses in 3 days (by applying little cream at the same time). In more serious cases of eczema, certain doctors prescribed the cream Élidel de Novartis (Swiss). Bad products for bad diagnosis? the hydrocortizone will not be appropriate inevitably for rosacée, and the creams against rosacée containing métrodinazole will not be appropriate for eczema. In the case of the seborrheic dermatite, a little sun does much good to the skin (it is an index for the diagnosis!) while it will not be appropriate for a skin reached of rosacée. The water of hamamélis is astringent which will make good in the event of blotches but which catastrophic in the event of will be rosacée or of eczema.

All the subjects reached of rosacée do not have the same symptoms. This disease being seriously studied only since relatively little time, research is not categorical as for the fact that it is indeed about only one and even disease in all the cases. One distinguishes however four sub-types from rosacée:

  • the erythematotelangiectasic rosacée : specific or persistent rednesses, appearance of blood-vessels on the surface of the skin. What goes sometimes until puffiness and a feeling of burn or heating.
  • the rosacée papulopustulaire . The skin of the face, generally red-faced woman, is invaded papules or pustules which make think of acne (but without black spots) and that accompany by unpleasant feelings of itchings and heating.
  • With the phymateuse rosacée , the skin thickens, generally on the level of the nose.
  • the ocular rosacée irritates the eyes and an appearance larmoyante gives them or injected blood. In the most serious cases it can result from it from the damage to the cornea even a loss of the sight.

Sensitive ground

This affection strikes the populations originating in the north of Europe, and particularly the Irish, Scottish, English or the Scandinavian - one speaks sometimes about “curse of Celts” - the Flemish, the Baltic , the Russian … Même if the fact is rare, rosacée can also touch African or a asiatique.
La rosacée would touch 45 million people in the world. According to studies, 10% of Swedish and 5% of American suffer from this pathology. It is estimated in addition that 70% of the subjects touched by this affection will always be unaware of to be reached of this disease.
Le diagnosis is often made starting from the thirty years age, often between 40 and 50 years. It is extremely rare that rosacée either diagnosed before the age of 20 ans.
Although the women are touched, the manifestations of rosacée generally take their acutest form at the men.

An uncomfortable disease

Rosacée of as much is badly lived by the subject than she is badly studied and badly known of the doctors non-specialists who often prescribe simple soaps antibactériens, thinking of being in the presence of a defect of hygiene or a problem of Alcoolisme. A survey shows besides that 50% of the people reached of rosacée wash the face with clear water exclusively, by fear of the symptoms which could exacerbate of other types of soins.
Des Canadian studies (with the the United States, the Canada is undoubtedly the country where rosacée is best studied currently) seem to show than the pushes of rosacée gear down the professional absenteeism and decrease the social life and self of those estimates which are exposed there.

Factors causing of rednesses

These factors depend on the patients who all are not also sensitive to the same things.
  • alcohols, and very specifically the red Wine.
  • hot drinks. Coffee and a long time were wrongfully shown, then one realized that it is their temperature which has an influence on the pushes of rosacée.
  • the fact of not drinking sufficiently.
  • an exposure to a great cold or hot summer dayss, the exposure to the sun. High winds. To take too hot baths.
  • physical effort.
  • the stress, anger, the embarrassment, tiredness, and all that is likely to make assemble the blood-pressure.
  • Certain affections: fever, colds, gastrite, diaphragmatic hernia or puffed out heat related to the Menopause.
  • the fact of scraping the face is also very harmful. The shavers are preferable with the blades.
  • products for the skin containing Oil, of oil of Eucalyptus, alcohol, hamamélis, Salicylic acid, menthol, Mint peppered or gasoline of Clove. Very produced for the skin which causes a drying or tinglings. To avoid the after-shaves.
  • Certain drugs such as the vasodilators (which dilate the blood-vessels) and the Stéroïde S topics.
  • Divers food has the reputation to cause pushes of rosacée: the spiced food, the Pepper, the Vanilla, the been useful food very hot, rich foods in Histamine, the Vinegar, the Tomato S, the Citrus fruits S and the Grape S but perhaps also the Chocolate, the sour cream, yogourths, the cheese, the eggplant, lawyers, spinaches, bananas and of many varieties of leguminous plants.

Treatments

Rosacée cannot be cured, but its demonstrations can be controlled.

Food

See “factors causing of rednesses” to know the list of disadvised food.

Hygiene

The subject often has the impression that to wash the face can cause crises of Acné, and it is a vicious circle because the lack of hygiene causes the bacterial attacks mechanically to which the couperosée skin is particularly sensible.
Il is necessary thus to frequently wash the face, but to as prevent as this care causes a drying of the skin, because such a drying will cause rougeurs.
The acne caused by rosacée has nothing to do with the teenage acne, it has there neither lubricating skin (séborrhée) nor pushed comedones. The products against the teenage acne are thus completely unsuited and can even exacerbate the symptoms of rosacée.

Hydration

The hydrating products fulfill their role very well, and it is important to use some after being washed the face. Rosacée a drying of the skin causes indeed, which reduces the resistance of the skin to the agressions.
The hydrating creams must be containing water, planned for one long life, without lanolin and parfum.
More natural, the fine sections of Cucumber, applied to the skin, have a hydrating effect and very effectively calm the cutaneous itchings associated with rosacée.

Medicamentous treatment

In the event of bacterial attack, creams Antibiotique S (in particular containing métronidazole, like the cream Rozex or products such as MetroGel , MetroCrème and MetroLotion ) can be prescrites.
Des antibiotic oral examinations can be prescribed, often successfully, but their prolonged use is déconseillée.
Des steroids topics can have a short-term effect but in the long run they have a tendency to worsen the symptoms of the blotches.

Lasers

A treatment the laser (not refunded by the social security in France) allows dévasculariser the skin of the patient during a few years (2 to 3 years). This method is very effective but it must be repeated regularly and remains relatively expensive.
