Osteopathy
The osteopathy is a nonconventional Médecine created by Andrew Taylor Still, (1828 - 1917) primarily based on manual techniques. The basic assumption is that any “mechanical” disturbance of the body (deterioration of the mobility of the articulations) involves effects on the total operation of the body, energy of located pains, articular, until the migraines and with the Infectious illness. As of the creation of osteopathy, the American osteopaths divided. A mobility saying " cultiste" and the other saying " scientifique" emerged. The latter was essential quickly on the United States but mobility known as " cultiste" prevail in Europe.
Definition
Osteopathy is based on four concepts:
- the unit: that of the body (all the parts are connected, by the Vascularisation, the Nervous system, the conjunctive Tissu), but also of the living being: the human being constitutes a whole, gathering its aspects physique, emotional, mental, intellectual, spiritual.
- the interrelationship enters the structure and the function: supposing that a body, even unfulfilled of an organic pathology, can function correctly only if its three-dimensional configuration is in conformity with that which was formed in the beginning, and conversely.
- " The role of the artery is absolu" meaning that, according to the osteopaths, any somatic structure unfulfilled of an internal injury is able to function normally, for little that its vascularization is correct - and that the food provided Nutriment S qualitatively and quantitatively sufficient.
- capacity of autoguérison: evoking the fact that the human body has all the capacities of adaptation and defense necessary. The osteopaths base their reflection on the conditions which put these means at fault, and try to raise the obstacle.
These concepts are applied to the anatomy and physiology human.
The text " Reference frame Ostéopathe" Profession; , established by five osteopathic associations jointly with the public authorities, recognizes three practices the ostéopathiques ones: that known as structural (including vertebral handling), the visceral osteopathy (which interest the bodies) and cranial osteopathy. This is not a recognition of their scientific validity.
Manual palpation seeks the somatic dysfonctions within all fabrics of the body. Apart from the interrogation, the Main represents the essential tool as well diagnosis as treatment.
The French government proposed an administrative definition of osteopathy that here:
- " Osteopathy and chiropractic constitute a whole of manual practices having for goal to identify the dysfunctions of mobility of the body and to cure it by techniques appropriées."
Osteopathy is addressed primarily to the system musculosquelettic and two other under-fields imagined by certain osteopaths and consistent in an alleged visceral and cranial system. The action on these various systems would pass by a major anatomical bond: the nervous system. Indeed all the techniques would have like common point to emit information with the related elements of the peripheral nervous system and vegetative nervous system (sympathetic and parasympathetic). This would explain why the action of osteopathy on the body is not local, but would touch the whole of the organization. Although nonspecific and not shown, the osteopathic techniques would thus have a beneficial action on the symptoms of affections concerning the fields according to:
-
the musculo-skeletal system: osteoarthritis, rheumatism, lumbago, sciatic nerve, periarthritis, distorsion, stiff neck, neuralgias, traumatisms…
- the system " viscéral" : constipation, congestion of the liver, gastrite, distensions, chronic cystitis, renal problem…
- the system " crânien" : nervous breakdown, chronic otitis, giddinesses, certain buzzes, stopped nose, chronic sinusitis, strabism, bad dental position, regurgitation of the infant, children difficult who often cry, insomnia, dyslexia and problems involved in the training, memory, concentration, migraine, cephalgia, facial neuralgia…
Decrees published recently strictly exclude teaching from these the last two fields highly discussed in the schools of osteopathy.
Effectiveness
To date, very few scientific studies evaluated the effectiveness of osteopathy. They are sporadic and their sometimes contradictory results.A study published in the New England Journal off Medicine and resumed by the Daily of the doctor French , shows the effectiveness of the treatments osteopathic in the cases of lumbagos subaigües (patient suffering since more than three weeks but less than six months).
Statute according to the countries
Belgium
The speciality is recognized since 2007, and titrates it allotted to the people having followed a formation of osteopathy.
Canada
The regulation depends on the provinces. In Ontario, the practice falls under the statute from the " experts not prescribing a médicaments". In Saskatchewan, osteopathy is regulated and reserved to the American osteopaths and to the Canadian doctors. With the Quebec, there does not exist yet of regulation. In the other provinces, the American osteopaths are only authorized to ask for an accreditation.
France
Since the law n° 2002-303 of the March 4th 2002 relating to the rights of the patients and the quality of the health system which in its article 75 recognizes the title of osteopath and chiropratician, osteopathy is not illegal any more in France. The decrees miss then which govern the conditions of exercise, in particular the formation. The crucible of practices and the differences of professional statute (doctors, kinesitherapists, non-kiné not-doctor) led to negociations tightened between various representative associations. It is only on March 27th, 2007, after several demonstrations of students and a recourse in Council of State, that the decrees appear with the Official journal. The osteopaths will have a regulated practice now and could be consulted in first intention. Nevertheless, the practice of handling of the cervical rachis as well as the practice of handling of cranium, the face and the rachis in the infant of less than six months will require a diagnosis established by a doctor attesting the absence of medical counter-indication to osteopathy.
The selection committee commission d'agr3ement of the establishments of training in osteopathy returned its conclusions in the month of August 2007. A commission of correction was held on September 4th, 2007. The establishments authorized to deliver a diploma of osteopathy are at the present time 19 (September 2007), while waiting for possible recourse on behalf of the not approved establishments.
For the health professionals the formation is approximately 1200 hours of theoretical teaching and practice of osteopathy. It is 2660 hours of teaching of fundamental sciences, biology, the theory and the practice of osteopathy in initial training for the titular people of a baccalaureat,
Osteopathy is always rejected by the Académie of medicine, only authority whose point of view is only medical, contrary to the Ordre of the doctors which is not a scientific advice. Osteopathy is not recognized as such like medicine in France, only like additional discipline.
Other countries
The the United Kingdom as well as the Suisse recognize this speciality exerted by various experts of all labels. The formation is university with the the United Kingdom and the osteopaths, recognized since 1993, are composed with more than 90% not-doctors, not-kinesitherapists. Only the experts recorded with the Order of the Osteopaths (GOsC) can carry the title d'" ostéopathe" and continuing education is obligatory.
The Sweden, the Norway and the Finland recognize since 1994 a joint base of medical studies. The students can then choose an official formation in a nonconventional medicine.
With the the United States, the osteopaths are doctors who carry the title of Doctor in Osteopathy (D.O.). They are formed in Ostéopathique medical schools, and they have the same rights as the doctors of medicine (Medical Doctors- M.D.s), without being able to carry the Is title of it has D.O year M.D?.
