A form , of “formula”, indicates in the beginning a collection of ordered scientific formulas. The legislative meaning is that of a model of legal document, which is close to the direction running of form: a standard “act” being used as model, general.
The first forms were those published by the notaries and the chancelleries. Nowadays, the form often refers to a type of general administrative document, to very often fill by the user.
