Dissociative disorder
A dissociative disorder consists of the disturbance of normally integrated functions. Diagnosis
Predisposition
The great majority of the subjects concerned with this disorder lived one traumatic, more particularly concerning sexual abuses. The vast majority of the people suffering from this Parasomnie suffer or already suffered from a psychiatric disorder.
Clinical aspects
It consists of episodes of behavioral problem night, corresponding in dissociative states (cf will infra) categorized well, or less. The clinical demonstrations are varied:- night Ambulations, runnings away night
- Cries, moanings, howls
- Behaviors hétéro and car-aggressive, being able to go until the suicidal gesture
- unsuited sexual Behaviors
- unsuited Behaviors of food
- …
At much of subjects, the night behaviors can be the acting out of a traumatic event. An episode can last of a few seconds to several tens of minutes, and be repeated several times per night. It occurs either shortly after drowsiness, or shortly after a night alarm clock. Similar episodes can also occur during the day at these same subjects.
There always exists, either an amnesia of the episode to the alarm clock, or a recollection of the night events in the form of a recall of Rêve (impression to have made a Cauchemar).
Complementary examinations
The Polysomnographie is essential to the diagnosis, it makes it possible to eliminate the differential diagnoses which are:- the Parasomnie S of the major slow sleep (Sleepwalking, night Terror, confusional awakening).
- Behavioral problem in paradoxical sleep
- complex forms of night epilepsy
Treatment
The assumption of responsibility is not codified, it requires a multidisciplinary intervention, in particular on the psychiatric level.
