During the first two years of its reign, regency was ensured by Raymond III of Tripoli, then it became major. It was the beginning of a reign astounding, being given, not only the age of the heir, but especially the disease which corroded it: the Leprosy. Baudouin and Saladin concluded then a truce in 1180, essential for the exhausted kingdom.

The efforts of Baudouin to maintain the peace and the existence of the kingdom were however decreased by two things:

  • the problem of its own succession: the heiress was Sybille, her sister, married in second weddings with Guy de Lusignan, a knight particularly stripped of political direction. In 1183, the leprous King organized his succession, initially entrusting the government to his brother-in-law, Guy de Lusignan, who appeared inefficient. He allotted then regency, in the name of his nephew Baudouin V of Jerusalem, with Raymond III of Tripoli. The difficulties of the succession inaugurated a succession of arguments which were going to severely shake the Latin kingdom of the East.
  • acts of armed robberies of Renaud de Châtillon which threatened fragile peace between the kingdom and Saladin.

Baudouin died the March 16th 1185 at the 24 years age of the ultimate phases of leprosy, leaving the kingdom with his nephew, still child. He was buried in the basilica of Holy Sepulchre, with dimensions of the other sovereigns of Jerusalem.
